Five facilities climbed up a grade. Six slipped down. Fifty-six stayed put, per the report that came out Nov. 13. One facility scored a D: CareWell Health Medical Center in East Orange. No facility in the state got a failing grade.
The Leapfrog Group hands out grades from A to F. They rate hospitals on how well they shield patients from medical mistakes, accidents, injuries, and infections. The group pulls 30 measures from public data and patient surveys to judge each facility.
“We are proud of the commitment to transparency and safety shown by our New Jersey hospitals,” said Adelisa Perez-Hudgins, vice president of quality at the Health Care Quality Institute of New Jersey, which partners with The Leapfrog Group, per NJ.com
Seventeen facilities in the state have racked up consecutive A grades for at least five rounds. Community Medical Center made the list. So did Englewood Hospital and Medical Center, Hackensack University Medical Center, Jersey Shore University Medical Center, and Morristown Medical Center.
Medical errors, accidents, injuries and infections harm one in four hospital inpatients. They cause as many as 250,000 deaths each year, per The Leapfrog Group. A 2016 national study discovered one in four patients were harmed by errors.
“The Leapfrog Group was founded 25 years ago to improve American health care through transparency, and the Safety Grade has been a cornerstone of that effort,” said Leah Binder, president and CEO of The Leapfrog Group.
Utah snagged first place for the fifth straight grading period. The top five scoring states are Utah, Virginia, New Jersey, Connecticut, and North Carolina. Four states earned no A grades: Iowa, North Dakota, Vermont, and Wyoming.
The organization examined health system performance for the first time in its history. The analysis revealed that 90% of facilities with a Fall 2025 Safety Grade belong to a health system. Among A-rated facilities, 94% are system-affiliated.
HCA Healthcare topped all systems with 51 A-rated facilities and 18 consecutive A performers. Chicago-based CommonSpirit Health placed second with 49 A grades and 24 consecutive A facilities.
