Chicago police say the incident unfolded just before 7 p.m. in the 3100 block of North Kimball Avenue in Avondale as the victim, a 32-year-old man, was driving.
As the man drove south along Kimball, officers said a light colored pickup truck with two unknown male offenders inside approached and opened fire on him before fleeing the scene.
The victim sustained a gunshot wound to his head amid the gunfire and eventually crashed his car into a residence in the area.
The victim was later taken to the hospital, where he was listed in critical condition.
Officers said the home that was hit in the crash was damaged, although the extent of the damage is unknown. However, no one inside the home was injured.
Currently, it is unclear what led to the shooting and an investigation is now underway.
