Call Rafael: Nursing rehab center faces loss of state license

INDIANAPOLIS — Tranquility Nursing and Rehab was forced to close and send patients to other facilities last month.

The center located at 3640 Central Avenue in Indianapolis is on the path to possibly get its state license yanked, after a highly critical review questioning its care of people.

On top of that, workers who fed patients and kept the building clean were among those who reached out to #CallRafael to complain about not receiving their paychecks.

“Pay me what you owe me, I did the work,” said Charlotte Wooden, who worked as a cook.

Wooden and other employees met FOX59/CBS4’s Rafael Sanchez outside Tranquility to express their frustrations as they scrambled to delay bills as they waited to get their pay.

“We all need paid what we are owed,” said Deanna Yaley who was a manager in food services.

Yaley and Wooden and Jimmy Williams who helped keep the building clean, had several attempts to reach the center’s leader, Ashok Mohan.

They say their texts and calls went answered. Sanchez encouraged them to try reaching him one more time. They got no response.

Sanchez sent numerous emails and texts to Mohan.

He was forced to close after the Centers for Medicare and Medicaid Services terminated its provider agreement on July 14. 

On a different front, the Indiana Department of Health has filed for a license termination.  We obtained a copy of the most recent review of the facility on June 27th.

71 pages list numerous problems questioning quality of care. The report on page three says the center “failed to ensure adequate supplies for residents.”

The review also pointed out that the nursing staff “rinsed out the feed bags to reuse.” On page 14, the review team highlighted concerns that the nursing staff  “disappears” by going “outside and either smoke or stay in their car for long periods of time.” 

Ashok Mohan did send me a brief text on Monday, August 4, at 2:49 p.m., saying that the money for the former employees would be in the bank to cover paychecks.

On Tuesday, all the people featured in our story had their checks clear.

They received pay for the first two weeks of July. Their final check covering the final two weeks of July is due at the end of this week.
