16-month-old dies from cardiac arrest while at preschool
Creative Beginnings of Lenoir was forced to close by the North Carolina Department of Health and Human Services two days after the toddler’s death. Daycare employee Alexandra Lee Coffey also was arrested and charged with involuntary manslaughter.
On Tuesday, DHHS officials told Queen City News that: “At this time, there is no longer a pending appeal of the summary suspension issued to Petitioner’s facility on May 21, 2025.”

The state’s suspension order has been effective during proceedings to suspend or revoke the facility’s license or during proceedings to cease operation.
A 911 call from that day revealed the desperate efforts of Creative Beginnings workers after finding Maddy Amara-Serenity Mitchell unresponsive. CPR was attempted, and emergency services were called, but Maddy was pronounced dead at a hospital that afternoon.
While the facility had passed its most recent inspection, records show previous violations related to safe sleep practices and incomplete staff training. During an unannounced visit in August 2024, DHHS reportedly officials discovered several violations including, “Bed, cribs, playpens, cots or mats were not placed at least 18 inches apart or separated by partitions when in use.”
