The clinic was opened in September 2014 in partnership with Rockford Public Schools, but Crusader said since then, RPS has provided transportation for students to receive medical, dental, and health education during the school day.
The clinic will relocate to 1200 W. State Street and the practice at Auburn will shut down.
“Crusader has provided quality pediatric primary care at Auburn for more than a decade. Our commitment to excellent comprehensive care is consistent,” said Crusader President and CEO Sam Miller. “We continually evaluate changing patient and community needs. Both Crusader and RPS 205 have changed and grown in the last 10 years. Despite the difficult decision to move our Auburn practice to West State, I am grateful for the partnership and optimistic for our future.”
