IDPH releases 4th Quarter 2024 Nursing Home Violations

CENTRAL ILLINOIS (WCIA) — The Illinois Department of Public health has posted a violation report for the last quarter of 2024, which includes 37 Central Illinois nursing homes.

IDPH announced the 2024 Fourth Quarter Report of Nursing Home Violations on Thursday. In total, there were 305 violation reports completed, and the full list can be found on IDPH’s website.

The Central Illinois nursing homes listed with violations include:

  • Accolade HC of Paxton on Pells
    • 1 Type B violation — $2,200
  • Accolade Healthcare of Danville
    • 1 Type A violation — $25,000
  • Accolade Healthcare of Pontiac
    • 1 Type B violation — $2,200
  • Accolade Healthcare of Savoy
    • 1 Type B violation — $2,200
  • Aperion Care Capitol
    • 2 Type C violations — $500
  • Arcadia Care Danville D/B/A La Bella of Danville
    • 1 Type B violation — $2,200

In total, Central Illinois had 37 nursing homes and 77 violations totaling up to $472,100 on IDPH’s report. Additionally, Accolade Healthcare of Savoy and Autumn Fields Savoy both received yellow card reports from the Champaign-Urbana Public Health District for multiple repeated violations.

Accolade Healthcare of Savoy’s inspection came on Monday and found six repeated offenses, including soiled items: fryers, the top of the dish machine and the countertop under the microwave. The Champaign County Public Health Department is ordering them to submit a long-term action plan for milk and ice-service units.

The inspection of Autumn Fields Savoy occurred on Wednesday and found nine repeated offenses, including marinara sauce that was in a cooler past the 7-day use period. This was the second consecutive inspection this exact violation had been documented. The health department is asking them to return a long-term corrective action plan for their “cold-holding” violation by Feb. 17.
